Supervisor, Integrated Service Delivery System – Temporary Full-Time Full Time

Pay Range

Base Salary: $70,000.00 – $87,000.00 (commensurate with experience)

This is a Temporary Full-Time role with a contract ending on October 31, 2026.

What will be your role?

The Supervisor, Integrated Service Delivery Systems role is a unique leadership position that supports OHTs’ strategy to improve client transitions through Hospital-to-Home and other integrated care programs like the Leading Practice (LP) projects that involve linking hospital and community partners to support earlier discharges of adults with higher needs to reintegrate into their home and community, within an Integrated Care model. The Supervisor, Integrated Service Delivery Systems blends clinical expertise with operational oversight, ensures coordinated scheduling, coaches an integrated care approach within the team and across the partners, and ensures ongoing communication with program funders, primary care, Ontario Health atHome (OHAH) and other community services. This leadership position will provide direction and operational oversight to both regulated and unregulated health professionals and may have shared supervisory accountability for rehab, nursing, personal support and homemaking roles. #LI-MD1 #INVHACorp

Key areas of accountability:

• Provides supervision, clinical leadership, advice, consultation, mentorship to an interprofessional and integrated home healthcare team across different funding models.
• Provide practice oversight to the assigned members of the team and ensures regular communication with partner organizations.
• Collaborate with leadership teams and clinical practitioners to promote client safety and risk reduction.
• Supports safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
• Acts as a key change agent in supporting organizational change.
• Function as a member of the Regional and Client Services teams and contribute to team and service effectiveness.

Key skills, experience and behaviors required for this position:

• Bachelor’s degree in a relevant healthcare discipline required; Master’s degree preferred (Preference will be given to candidates with a Graduate Degree in Nursing, Education or Health Administration)
• Must be a regulated healthcare professional in good standing with their respective college or regulatory body
• Ongoing membership with professional associations an asset.
• Experience in a supervisory or advanced practice role is an asset.
• Good understanding of the health care services and delivery processes in the community and home care sector.
• Experience working within an interprofessional or integrated care approach is essential.
• Knowledge and experience with gerontology
• Intersectoral experience and excellent relationship building skills and dispute resolution
• Demonstrated commitment to client and family centered care.
• Strong clinical practitioner with an ability to use knowledge-transfer techniques to put evidence-based knowledge into practice.
• Excellent leadership and facilitation skills and the ability to coach and motivate teams.
• Demonstrated knowledge and/or proficient skills in the use of information technology for electronic clinical documentation and clinical decision support.
• Awareness and understanding of all relevant legislation related to practice standards, standards of safety and quality, and employment standards including human rights legislation.
• Demonstrated leadership skills with strong organizational and self-management skills, and with demonstrated ability to achieve personal, professional, and organizational performance targets.
• Superior interpersonal, verbal and written communication skills, and skilled in customer service.
• Clear, logical, system thinker and innovative problem solver with demonstrated advanced judgement and decision-making skills.
• Demonstrated ability to work independently, and to lead or contribute as a member of an interprofessional team.
• Demonstrated commitment to modelling lifelong learning and continuous professional growth.
• Demonstrated knowledge of project management approaches and tools
• Knowledge and experience in use of standardized assessment tools in care plan development an asset.
• Use of proper body mechanics and successful completion of the Physical Readiness Test to ensure safe work practices are followed, including mobility, transfers, bending, lifting, pushing and pulling 
• Availability to participate in VHA’s On-Call roster.
